Electronic Record and Signature Disclosure
This disclosure is from UCPlaces LLC, doing business as DentalSlip, 303 Maple Ave W, Suite F, Vienna, VA 22180, info@dentalslip.com. It describes the electronic records and electronic signatures used when someone signs a document through DentalSlip. It is written for the person signing.
Records this covers
Your consent covers the document the office sent you to review and sign through DentalSlip, the record of that signature, and the completion certificate stored with the signed document. It also covers emails DentalSlip or the office sends about that document, such as a signing code or a copy. It does not cover a different document unless you agree again for that document.
Paper copies
You can ask for a paper copy of a document you signed, or of this disclosure. Contact the office that sent the document. That office sets any fee for a paper copy and tells you how to request it. DentalSlip does not decide the office's fee.
Withdrawing consent
You may withdraw this consent before you finish signing. Close the page, or decline on the signing page and tell the office. Withdrawing consent means you will not sign that document electronically. The office may then ask you to sign on paper, or it may be unable to complete the request. Withdrawing consent does not undo a signature you already applied. To withdraw after you have signed, contact the sending office. DentalSlip cannot change the office's document.
Updating your contact information
Tell the sending office if your name, email, or phone number changes. The office uses that information to reach you about the document. DentalSlip displays the contact details the office provides.
Hardware and software
You need a current version of a common web browser with JavaScript and cookies enabled, a screen that can show the pages, and a way to open a PDF if you want a copy. You also need an email account if the office sends a code or a copy. A phone, tablet, or computer that can run that browser is enough. No special hardware is required.
Who is responsible for the document
The office that sent the document is responsible for its content. DentalSlip provides the signing service. DentalSlip does not prepare, review, or approve the document itself.
Questions about this disclosure can be sent to info@dentalslip.com. Questions about a particular document should go to the office that sent it.
