How to put a referral form on your website

The simplest way to accept referrals is a link you can text or email. The form opens on DentalSlip, shows your office name and phone, and sends the referral to your inbox.

If you want the form on your own website, paste an iframe into the page. On WordPress, a Custom HTML block is enough. The form stays on DentalSlip’s address, inside the frame, so the surrounding website does not receive patient answers, attachments, or the PDF.

That separation matters. A plugin on the marketing site should not be able to read what a referring office typed. If the website is ever compromised, the frame can still be replaced, which is why a direct link should stay available as a fallback.

Publish only after a notification email is confirmed. The message that goes out says a referral is waiting. It does not include the patient’s name, the reason, or an attachment.

A WordPress plugin and a script loader are planned later. The iframe and the direct link cover the first release.
