Design the form your referring offices will fill out, put it on your website, and read every referral in one inbox. Referring offices are not charged and do not create an account.
The canvas is the form. Place one, two, or three fields on a line, preview a draft, and add your logo and letterhead. Start from Periodontics, Endodontics, Oral surgery, Orthodontics, General, or a blank form. Save and duplicate templates.
A tooth chart uses universal numbers 1–32. The person filling out the form can tap a tooth or type the numbers. The same chart is drawn on the PDF.
Copy a link or an iframe. On WordPress, a Custom HTML block is enough. The form stays on DentalSlip’s address, so the surrounding website does not receive the answers or the files.
Referrals arrive as New. Your team can assign them, add private notes, and move them through review. A badge shows how many are still New. Billing-only staff cannot open a referral.
A referring office does not create an account and is not charged. Double-clicks do not create two referrals. A receipt is issued only after the referral is saved. Paper still works: print a blank form, print without submitting, or submit and print.
A Business Associate Agreement is included. Forms stay unpublished until it is accepted, the subscription is active, and at least one notification email is verified. Patient names stay out of that email, the web address, and the card processor.
The customer is the office that receives the referral. A periodontist, endodontist, oral surgeon, orthodontist, or a general dentist who takes referrals can subscribe on the same terms. One physical location is one subscription. A person can belong to more than one office and switches between them on purpose.
The periodontics template follows a NovaPerio-style referral: date, patient name, date of birth, phone, insurance, referring dentist, referral reasons, and comments. Crown lengthening, grafts, implants, and maintenance are already listed. You can rename or remove anything.
