E‑signatures

Send a document for signature, publish a form a patient can open on their own, or hand someone a tablet in the office. You prepare the file, place the fields, and send a link.

E‑signatures are available for an office once an owner or administrator accepts the Business Associate Agreement.

It works like Dropbox Sign: upload a document, place the fields, and send a link.

Any document, including Word

Upload a PDF, a .docx, or a Word 97–2003 .doc file. Word files are converted to PDF before you place fields. Each signer has a color on the page.

Self-serve forms

Publish a form, such as patient intake, and share a link or an iframe. The patient enters their own name and email and signs. A published form uses one live form, shared with referral forms. A document you only send does not use a live form.

In person on a tablet

Start a signing session at the front desk. The person signs on that tablet. You can ask someone at the office to countersign afterward.

Consent first

The signer agrees to electronic records before any field can be filled in. That agreement is for this document. A different document needs agreement again.

Audit trail and completion certificate

When everyone has signed, a completion certificate is appended to the PDF. It records the consent time, the disclosure version, the IP address, the browser, and whether the signer agreed to an email copy. The same completion is written to the office audit log.

Branded emails

Signer emails use the practice name. When the office has uploaded a logo, that logo is in the email. If there is no logo, the DentalSlip tooth is used.

Sending is unlimited on every plan. Each file is limited by the plan, measured on the PDF after a Word file is converted. Basic allows 40 MB and 100 pages per document. Pro allows 100 MB and 500 pages per document. Group allows 250 MB and 1,000 pages per document.
